Acc long instructions for use. Instructions for use acc long

ACC Long belongs to the group of mucolytic drugs, which are highly effective in the treatment of pathological conditions of the airways with difficult-to-separate mucus. In addition, for people suffering from chronic bronchitis and cystic fibrosis, this pharmacological agent is prescribed from a prophylactic point of view to relieve the frequency and severity of the exacerbation stage. As the instructions for use of ACC Long 600 mg indicate, there are some restrictions regarding the use of tablets. Therefore, before using, you must consult a doctor for advice, which will establish true reason diseases and develop an individualized treatment plan.

Composition and dosage form of ACC Long

ACC cough medicine is presented in a pharmacy in tablet form, easily soluble in water. The product is released in 6, 10, 20 pieces in one tube, placed in a package made of thick cardboard.

Thanks to acetylcysteine, the main substance of the drug, when it is used, a pronounced mucolytic effect is observed, due to which the thick pathological secret becomes as liquefied as possible and is easier to remove from the air channels. The principle of operation lies in the ability of the active component to block the disulfide bridges of mucopolysaccharide chains and activate the depolymerization of the mucoids of sputum formations. The drug ACC Long also shows its effectiveness in purulent formations.

In addition, acetylcysteine has an antioxidant effect. The active substance also has a beneficial effect on the synthesis of glutathione, one of the main components of the antioxidant system and the process of cleansing the body with chemical means.

Due to the antioxidant activity, the degree of protection of cells from the harmful effects of free radical oxidation, which is inherent in a strong inflammatory lesion, increases.

One tablet medicinal product cough contains 600 mg of the main ingredient - acetylcysteine.

White tablets have one side with a flat surface, and the other with a dividing line in the center. To prepare the medicine for use, it is enough to dissolve 1 tablet in a container with 100 ml of warm water. A colorless liquid with a pronounced berry aroma.

Pharmacokinetics

ACC Long stands out with a high degree of absorption, is very quickly cleaved in the liver and creates pharmacologically active metabolic products - cysteine, diacetylcysteine, cystine and mixed disulfides. The indicator of the ability of this medication to be absorbed when ingested is estimated at 10%, this is due to the strong effect during the first penetration through the liver. Level max high rate in blood plasma observed after 1-3 hours. The percentage of binding to plasma proteins is 50%.

The output of unused metabolic products occurs through the kidneys. The half-life is not more than 60 minutes, in case of various disorders in the liver, this period is extended to 8 hours.

When using this medication, you should be aware of its penetration through the placental barrier. Information on the ability of the active ingredient to pass through the blood-brain barrier and be present in breast milk is not indicated.

Taking ACC Long is effective in pathological conditions of the respiratory apparatus, which occur with sputum secretions of a viscous concentration. It:

  • laryngotracheitis;
  • bronchitis of any form;
  • tracheitis;
  • bronchiectasis;

cystic fibrosis: as mentioned above. It is possible to use in children from 6 years old - one effervescent tablet once a day.
Mode of application
Dissolve the effervescent tablet in one glass of cold or warm water. The finished solution should not be mixed with other drugs. The drug can be taken regardless of food intake.

If you have overdosed on ACC LONG
So far, no severe side effects and signs of drug intoxication have been observed, even with a significant overdose.
If the dose is exceeded, irritation may occur. gastrointestinal tract(eg, nausea, vomiting, diarrhea). Children are at risk of bronchial hypersecretion (excessive sputum production). In case of a suspected overdose of ACC LONG, you should contact your doctor.
If you forget to take ACC LONG
If you miss one dose of ACC LONG or take too little dose, wait until the next dose and continue taking the drug as directed in the dosage recommendations. Do not double your dose to make up for a missed dose.
If you have any further questions about the use of this medicine, ask your doctor or pharmacist.

Contraindications

Do not take ACC LONG if you are hypersensitive (allergic) to acetylcysteine or any of the other ingredients of ACC LONG, or if you have an exacerbation of gastric ulcer or duodenum, galactose intolerance, lactase deficiency and malabsorption of glucose-galactose.
ACC LONG should not be used to treat children under 12 years of age (6 years for cystic fibrosis).

Special Warnings and Precautions for Use

When taking ACC LONG, it is not recommended to use antitussive drugs.
Due to the risk of bronchospasm in patients with bronchial asthma and bronchial hyperreactivity, caution is advised. If a hypersensitivity reaction or bronchospasm occurs, the use of the drug should be immediately discontinued and appropriate measures taken. When taking the drug, you should be careful if you suffer from bronchial asthma, if you have a history of or currently have a stomach or intestinal ulcer, as well as the risk of gastrointestinal bleeding(for example, latent peptic ulcer or esophageal varices), since taking acetylcysteine may cause vomiting.
Taking acetylcysteine, especially at the beginning of treatment, can lead to liquefaction of bronchial secretions (sputum). In case of violation of the ability to independently cough up (suffocate enough), appropriate measures should be taken to prevent dangerous stagnation of sputum - bronchial drainage and aspiration.
Acetylcysteine leads to inhibition of diamine oxidase (DAO) in vitro by 20-50%. Caution should be exercised in patients with histamine intolerance. Long-term therapy should be avoided in such patients, as ACC LONG interferes with histamine metabolism and may cause symptoms of intolerance (eg, headache, runny nose, itching).
During the use of acetylcysteine, in very rare cases, the occurrence of severe skin reactions, such as Stevens-Johnson syndrome and Lyell's syndrome, has been observed. In the event of changes in the skin and mucous membranes, you should immediately stop taking the drug and seek medical help.
The presence of a slight sulfuric odor, which may appear when the package is opened, is characteristic of the active substance. The smell quickly evaporates and does not affect the effectiveness of the drug.
The drug is not recommended for use during pregnancy and during breastfeeding.
One effervescent tablet contains approximately 6.03 mmol (138.8 mg) sodium. This should be taken into account when using the drug in patients on a controlled sodium diet (low sodium / low salt content).
Due to the presence of lactose in the composition of the drug, ACC LONG is contraindicated in patients with rare hereditary galactose intolerance, lactase deficiency or malabsorption of glucose-galactose.

Due to the presence of ascorbic acid in the composition of the drug use with caution in: deficiency of glucose-6-phosphate dehydrogenase, hemochromatosis, sideroblastic anemia, thalassemia, hyperoxaluria, oxalosis, nephrolithiasis.
Ascorbic acid increases the risk of developing crystalluria in the treatment of short-acting salicylates and sulfonamides, slows down the excretion of acids by the kidneys, increases the excretion of drugs that have an alkaline reaction (including alkaloids), and reduces the concentration of oral contraceptives in the blood.
Ascorbic acid can distort the results of various laboratory tests (blood glucose, bilirubin, transaminase activity, LDH). High doses of ascorbic acid can give false-negative test results for blood hidden in the feces.

Taking other drugs

Tell your doctor or pharmacist if you are taking/using or have recently taken/have used any other medical preparations including over-the-counter drugs.
The addition of other drugs to acetylcysteine preparations should be avoided. Simultaneous administration of acetylcysteine and activated charcoal during intoxication therapy may reduce the effectiveness of acetylcysteine.
The reports received so far about the ability of acetylcysteine or other mucolytics to inactivate antibiotics are exclusively experimental. in vitro in which the respective substances are mixed directly with each other. However, for safety reasons, oral antibiotics should be taken separately from acetylcysteine, with at least a 2-hour interval.
Simultaneous administration of acetylcysteine and nitroglycerin can lead to an increase in the vasodilating and antiplatelet action of the latter.
The simultaneous use of acetylcysteine and carbamazepine can lead to a decrease in carbamazepine concentrations to subtherapeutic levels. Acetylcysteine is incompatible with most metal preparations and is inactivated by oxidizing agents.
Ascorbic acid-related interactions
Ascorbic acid with simultaneous use reduces the chronotropic effect of isoprenaline. With prolonged use, it can disrupt the interaction of disulfiram-ethanol. Increases the toxicity of amygdalin when taken together. Reduces the therapeutic effect of antipsychotics (phenothiazine derivatives), tubular reabsorption of amphetamine and tricyclic antidepressants. Increases the concentration of benzylpenicillin and tetracyclines in the blood.
Improves intestinal absorption of iron preparations; may increase iron excretion when used concomitantly with deferoxamine, as a result of which ascorbic acid should not be taken during the first month of treatment with deferoxamine. Simultaneous intake of ascorbic acid and aluminum hydroxide may increase the absorption of aluminum.

Pregnancy and breastfeeding

Pregnancy
To date, there is not enough data on the use of acetylcysteine in pregnant women. Therefore, taking ACC LONG is not recommended during pregnancy and during breastfeeding.
Breast-feeding
Data on the ability of acetylcysteine to penetrate into breast milk is not available. Therefore, taking ACC LONG is not recommended during pregnancy and during breastfeeding.

mucolytic agent.

ATX code: R05CB01.

Pharmacological properties

Pharmacodynamics
Acetylcysteine is a derivative of the amino acid cysteine. It has a mucolytic effect, facilitates sputum discharge due to a direct effect on the rheological properties of sputum. The action is due to the ability to break the disulfide bonds of mucopolysaccharide chains and cause depolymerization of sputum mucoproteins, which leads to a decrease in sputum viscosity. The drug remains active in the presence of purulent sputum.
It has an antioxidant effect based on the ability of its reactive sulfhydryl groups (SH-groups) to bind to oxidizing radicals and thus neutralize them. In addition, acetylcysteine promotes the synthesis of glutathione, an important component of the antioxidant system and chemical detoxification of the body. The antioxidant effect of acetylcysteine increases the protection of cells from the damaging effects of free radical oxidation, which is characteristic of an intense inflammatory reaction.
With the prophylactic use of acetylcysteine, there is a decrease in the frequency and severity of exacerbations of bacterial etiology in patients with chronic bronchitis and cystic fibrosis.

Pharmacokinetics
Absorption is high. They are rapidly metabolized in the liver with the formation of a pharmacologically active metabolite - cysteine, as well as diacetylcysteine, cystine and mixed disulfides. Bioavailability when taken orally is 10% (due to the presence of a pronounced "first pass" effect through the liver). The time to reach the maximum concentration in the blood plasma is 1-3 hours. Communication with blood plasma proteins is 50%. It is excreted by the kidneys in the form of inactive metabolites (inorganic sulfates, diacetylcysteine). The half-life (T1 / 2) is about 1 hour, liver dysfunction leads to an extension of T1 / 2 up to 8 hours. Penetrates through the placental barrier. There are no data on the ability of acetylcysteine to penetrate the blood-brain barrier and be excreted in breast milk.
