What is Miramistin?

The drug was developed in the USSR back in the 70s of the last century. Since then, it has not lost its high efficiency.

Nonoxynol-9 is known as a spermicide with combined antimicrobial activity. The antiseptic methylene blue - methylmethionine sulfonium chloride - also has a quaternary nitrogen atom in its structure and, according to the literature, has a spermicidal effect. Experiments conducted to determine the spermicidal activity of antimicrobial compounds suggest that establishing the fact that a substance belongs to quaternary ammonium bases a priori presupposes their spermicidal activity and the possibility of use as local contraceptives with combined antimicrobial activity.

Benzalkonium chloride has the ability, when used topically, to damage sperm membranes, exhibiting spermicidal activity, while providing a pronounced antiseptic effect. It is the active ingredient of the imported drug Pharmatex, produced in the form of vaginal tablets, suppositories, creams and tampons by Innotek International. (France). The disadvantages of benzalkonium chloride include the allergic reactions it causes, itching and burning in the vagina, which are observed in approximately 8% of women, as well as contact dermatitis, urinary tract infections, candidiasis, vulvovaginitis and allergic vaginitis.

Ethonium - 1,2 - ethylene - bis - (N - dimethylcarbdecyloxymethyl) ammonium dichloride, which also belongs to the biquaternary ammonium bases, derivatives of ethylenediamine, is also of interest as a spermicide. Until recently, aethonium was used as an antimicrobial, anti-inflammatory and stimulating reparative processes agent for the treatment of cracked nipples, vulvovaginitis, pruritic dermatoses, proctitis and sigmoiditis. The spermicidal activity of etonium® has been confirmed in animal experiments. Being a structural analogue of benzalkonium chloride, aethonium exhibits spermicidal activity due to the above mechanism of action. The disadvantages of etonium include its lack of activity against the causative agent of syphilis (treponema pallidum), the lack of antifungal and antiviral activity.

Nonoxynol-9-mono(para-nonylphenyl) ether of polyethylene glycol is also known as a spermicide - a potent surfactant that has a quaternary nitrogen atom in its structure. This compound destroys the mucous membrane of sperm and at the same time has a bactericidal and virusicidal effect.

A well-known antiseptic drug from the group of dyes - methylene blue - methylmethionine sulfonium chloride - also has a quaternary nitrogen atom in its structure and, according to the literature, has a spermicidal effect.

Surfactant compounds with cationic activity include miramistin - mirastachloride, which has a pronounced antimicrobial effect against gram-positive (staphylococci, streptococci) and gram-negative (gonococci, shigella, salmonella, Escherichia, chlamydia, corynebacteria, mycobacteria, pseudomonads) bacteria, pale treponema, pathogenic fungi (yeast-like, ascomycetes, dermatophytes), some viruses (influenza A, herpes types 1 and 2, coronaviruses, adenoviruses, HIV-1) and protozoa (Trichomonas). It should be noted that in concentrations that have bactericidal activity, miramistin does not cause local irritation, mutagenic or carcinogenic effects. These properties of miramistin made it possible to recommend it for use in gynecology, in particular, for the prevention and treatment of syphilis, gonorrhea and a number of non-gonorrheal diseases of the genitourinary tract. The presence of such a wide list of indications for the use of miramistin determined the feasibility of studying its spermicidal activity.

We conducted a study to study the spermicidal activity of miramistin using cytomorphological methods on sperm from normal human ejaculate: the number of motile sperm, the number of actively motile sperm, the survival time of sperm were counted, the pH of sperm was determined before and after equilibration of sperm with aqueous solutions of chlorhexidine bigluconate. All studies were carried out in vitro on 15 human ejaculate samples corresponding to normal values. Initial data: number of sperm in 1 ml 78.8 ± 3.95x10 6, number of motile sperm 68.06 ± 2.37%, number of actively motile sperm 28.86 ± 1.09%, sperm pH 7.32 ± 1, 13, sperm survival time is 359.76±10.24 seconds, the number of morphologically normal forms is 78.33±0.12%, the number of leukocytes is 0.7±0.64 x10 6 /ml. After studying the native sperm, the studied aqueous solutions of Miramistin of various concentrations were added to each of the ejaculates: 0.05%, 0.06%, 0.07%, 0.08%, 0.09%, 0.1% As a domestic standard of comparison a 1% solution of quinosol, which has spermicidal activity, was used (control). After mixing native sperm and quinosol solution in equal quantities and subsequent application of the mixture to a glass slide, sperm in any form (motile, actively motile and live) were not observed. The effect of aqueous solutions of miramistin was assessed at the time of their addition to native sperm, 1 minute, 5 minutes, 10 minutes, 20 minutes, 30 minutes, 1 hour, 2 hours and 3 hours after incubation. The volume ratio of sperm and test solutions was 1:1. The obtained data were processed on a computer using the Statgraphics Plus software package. An illustration of research results reflecting the effect of 0.1% Miramistin solutions on sperm viability, expressed in absolute immobilization of sperm, is presented in Table 1.

Moreover, when mixing ejaculate samples and miramistin solutions of lower concentrations, viable sperm were observed in the field of view. The above indicates the prospects of using miramistin as a local contraceptive.

Summarizing the above, we can conclude that establishing the fact that a substance belongs to quaternary ammonium bases presupposes a priori its spermicidal activity. At the same time, official antimicrobial drugs with the appropriate chemical structure can be considered promising local contraceptives.

Antiseptics are used in many areas of medicine. This drug is used in dentistry, otolaryngology, surgery and many other branches of medicine. It also finds wide application in venereology. The antiseptic also destroys herpes and human immunodeficiency viruses.

Only women need to additionally treat the vagina. For this, a special gynecological attachment is used. It comes complete with the drug. Remove the nozzle from the sterile packaging. Then put it on top of the bottle without removing the urological attachment. After this, 5-10 ml of solution is injected into the vagina. It needs to be held there for several minutes.

A spray bottle can be used to treat thighs and other areas of skin. This is another attachment that comes with the kit. Before putting it on, you need to remove the urological and gynecological attachment.

It makes sense to use Miramistin only in the first 2 hours after sexual intercourse. And it is advisable to do this in the first few minutes. It's good if you have it in your home medicine cabinet. And if not, you need to urgently run to the pharmacy in order to have the genitals treated as quickly as possible. Then you cannot urinate for 2 hours.

Patients often ask whether prophylaxis is necessary if more than 2 hours have passed since sex. After all, the instructions indicate that it is carried out only during this time.

In fact, Miramistin may prove effective later. But the effectiveness of prevention is significantly reduced. During the first 2 hours, the drug gives maximum effect.
