PERMISSION WORK (work order) is a task for the performance of work, drawn up on a special form of the established form and defining the content, place of work, time of its beginning and end, conditions for safe conduct, composition of the team and persons responsible for the safe performance of work (Intersectoral (rules) safety) during the operation of electrical installations POT R M-016–2001, RD 153-34.0-03.150-00, approved by Resolution of the Ministry of Labor of Russia dated January 5, 2001 No. 3 and Order of the Ministry of Energy of Russia dated December 27, 2000 No. 163).

The procedure for issuing N.-D. and work on it is defined, for example, in SNiP 12-03–2001 “in construction. Part 1. General requirements" (approved by Decree of the State Construction Committee of Russia dated July 23, 2001 No. 80 to replace SNiP 12-03-99* with amendment No. 1, put into effect on September 1, 2001).

According to SNiP 12-04–2002 “in construction. Part 2. Construction production" (adopted by Decree of the State Construction Committee of Russia dated September 17, 2002 No. 123, put into effect on January 1, 2001) excavation work in the security zone of high voltage cables, existing gas pipelines, other communications, as well as areas with possible pathogenic contamination of the soil (landfills, cattle burial grounds, cemetery, etc.) must be carried out according to N.-d. after receiving permission from the organization operating these communications or the sanitary inspection authority. Work in these conditions is carried out under the direct supervision of the work manager, and in the security zone of live cables or existing gas pipelines, in addition, under the supervision of employees of organizations operating these communications.

N.-d. issued to perform work in areas of hazardous production factors, the occurrence of which is not related to the nature of the work performed. A list of places of production and types of work where it is permissible to perform work only in accordance with N.-D. must be drawn up in the organization, taking into account its profile, and approved by the head of the organization. , authorized by order of the head of the organization, issues N.-d. to the immediate supervisor of the work (foreman, foreman, manager, etc.) and monitors the implementation of the measures provided for in it to ensure the safety of the work. Before starting work, the work manager is obliged to familiarize workers with work safety measures and register them in the N.D.

When performing work in security zones of structures or communications, N.-d. may be issued with written permission from the organization that owns this structure or communication. N.-d. issued for the period necessary to complete the specified amount of work. If harmful and (or) hazardous production factors that are not covered by the N.-D. arise during the production of work, the work should be stopped and the N.-D. canceled. Work can be resumed only after the issuance of a new N.-D.

SNiP 12-03–2001 establishes an approximate list of places (conditions) of production and types of work for which it is necessary to issue N.D.:

performing work using cranes and other construction machines in the security zones of overhead power lines, gas and oil pipelines, warehouses for flammable or combustible liquids, flammable or liquefied gases;

performing any work in wells, pits, confined and hard-to-reach spaces;

carrying out excavation work in areas with pathogenic soil contamination (landfills, cattle burial grounds, etc.), in security zones of underground electrical networks, gas pipelines and other dangerous underground communications;

carrying out routine repairs, dismantling equipment, as well as carrying out repairs or other construction and installation work in the presence of hazardous factors of the operating enterprise;

performing work in areas where there is or may arise from adjacent work areas;

performing work in the immediate vicinity of the roadway or bed of operating roads and railways (determined taking into account the current regulatory documents on labor safety of the relevant ministries and departments);

performing gas hazardous work.

In accordance with GOST 12.0.004–90 SSBT “Occupational safety training. General provisions" when issuing N.-d. a target on occupational safety is carried out with employees, which is recorded in the N.-d. or other documentation permitting work to be carried out. carried out by the immediate supervisor of the work.

In N.-d. are indicated:

date of issue and validity period;

Full name of the work manager;

name of the work, place and conditions of its implementation;

a list of hazardous production factors that act or may arise regardless of work at the places of their production;

time, start and end date of work;

If work is carried out on the territory of another organization, then there must be written permission from the operating enterprise (operating organization) to carry out the work. A note is also made: “Measures for the safety of construction production have been agreed upon” (full name, position, signature of an authorized representative of the operating enterprise or operating organization).

Since August 2014, Law No. 328n comes into force. In accordance with it, a new edition of the “Rules on labor protection during the operation of electrical installations” is being introduced. Interindustry standards require managers of each enterprise where technological processes involve the use of electrical equipment to train personnel in the new rules and organize unscheduled knowledge testing. At the same time, the amount of information must be absorbed by employees in proportion to their responsibilities.

Structure of the new Rules

The content of the annex to Law No. 328n, naturally, has undergone some changes, but the names and meaning of the main sections have remained the same. This also applies to the content of the chapters, which stipulate the safety of work in electrical installations for which a permit is issued. The rules stipulate that it should occur after crew members have been given instructions on labor protection for workers, according to their specialties.

The first three sections of the appendix to Law No. 328n stipulate the scope of its application, requirements for personnel, operational maintenance and inspection of equipment and do not mention work on the side in electrical installations. Detailed conditions for issuance, registration procedure, qualifications of the admitter, observer and manufacturers are set out in chapters starting from V.

Based on these lists, and in their absence - on the basis of the specifics of the enterprise’s work, its profile, the employer is obliged to develop and approve its list of work performed according to the permit.

Work permit - this is a written order on a special form that determines the content, place, time and conditions of work, the necessary safety measures, the composition of the team and persons responsible for the safety of work. A distinctive feature of the work under this document is that even before it begins, at the stage of issuing a permit, based on the list of hazardous and harmful factors (not necessarily production) operating in the area, preliminary labor protection measures are developed and entered into the permit form ( which will allow relatively safe entry into the work area), carry them out and accept signature; determine the necessary means of collective and individual protection, strictly regulate the work to be performed, develop labor protection measures that must be carried out during work; Conduct, against signature, targeted instruction on safe work practices with team members.

The persons responsible for the organization and safe performance of work under the permit are: 1) the person issuing the permit; 2) work manager; 3) the responsible executor of the work and 4) members of the team (performers of the work).

The person issuing the permit, determines the content of the work to be performed and the conditions for its safe execution, appoints a work manager, a responsible executor and team members. He is responsible for the correctness and completeness of the safety measures specified in the work permit, for the adequacy of the qualifications of the responsible persons and team members appointed by him. A permit-permit has the right to be issued by a person authorized to do so by order of the head of the organization. In each enterprise, the manager approves by order a list of such persons to whom and for what types of work he instructs to issue permits. The work permit is issued in two copies: the first copy remains with the person who issued it, and he hands the second copy to the work manager. The responsible performer of the work does not have the right to accept the work permit and begin the work of the team (unit) if the nature and working conditions, labor safety measures do not comply with the current rules and instructions or are not reflected in the work permit to the required extent.

It is prohibited to start work before the time specified in the permit, or to expand the scope of work provided for by the permit. If it is necessary to perform additional work, as well as if harmful or dangerous factors arise during the execution of work that are not covered by the permit, the work should be stopped and another permit issued.

Workers (performers of work) have the right to be in the work area only at the time specified in the work permit and only in the presence of the responsible executor or work manager.

Upon completion of the work, their supervisor hands over the closed permit to the person who issued the permit and makes a corresponding note in the journal for issuing permits.

Among the main shortcomings in organizing the production of work according to work permits at enterprises, one can highlight: non-appointment by orders of the head of the enterprise of persons who are entrusted with issuing work orders, the absence of approved lists of work performed according to this document, and, as a consequence, the execution of these works without any preparatory activities and without issuing a work permit. Such violations are regularly identified during the investigation of fatal accidents that occur during the performance of work of this type. Thus, in one of the farms in the Tver region, while ensiling feed, a worker resting on the grass near the silage trench was crushed under the wheels of a car. During the investigation of this case, many violations were revealed: targeted training was not carried out with workers, safety measures were not developed, there were unauthorized persons in the danger zone, those responsible for the work were not assigned, there was no control over the work, the territory of permitted entry and exit of cars transporting green mass was not marked, i.e. exactly those activities that the permit was required to provide for were not carried out, but it was not issued.

Multiple cases of death of workers due to unauthorized expansion of the scope of work have been recorded. Thus, a team of electrical workers, according to a work permit, was ordered to install an overhead power line to a substation without connecting to the network. However, at the request of the director of the high school to supply voltage to a nearby school site under construction, two of the team members entered the substation room and, while trying to fulfill the request, one of them, as a result of erroneous actions, touched a high-voltage bus with his hand and died at the scene. One of the main causes of an accident is the performance of work not provided for by the work permit, and, as a result, without proper consideration of safety issues.

Carrying out work with the issuance of a permit certificate carried out in cases where the head of the organization invites any contractor to his territory to perform construction, installation, finishing, commissioning and other similar work. This is necessary to establish the boundaries of responsibility between the organization and the contractor for the safe performance of work.

The approval certificate indicates the location of the site allocated for the work, its coordinates (axes, marks) marked on the plan of a specific facility or organization, the start and end dates of the work, labor protection measures to ensure the safety of the work, the deadline for completing these measures, and the performers. The performers of all activities or some of them can be any party in accordance with the terms of the contract.

The approval certificate is signed by representatives of the organization and the contractor. If it is necessary to perform work after the expiration of the permit certificate, it is necessary to draw up a new permit certificate.

If an accident occurs during the performance of work documented in the permit certificate, then responsibility for it will primarily be borne by the party who, in accordance with the permit certificate, was obliged to ensure the safety of this type of work.

Drawing up an authorization certificate does not relieve the performer of the work from the need to draw up a work permit if there are harmful or dangerous factors in the area allocated for the work.

LIST OF HIGH-RISK WORKS FOR WHICH IT IS NECESSARY TO ISSUE A PERMISSION WORK WORK:

1. Electric-gas welding, gas-flame, surfacing and soldering work performed outside places specially equipped for this work.

2. Work at height (when performing work at a distance of less than 2 meters from unfenced differences in height of 1.3 m or more).

PROCEDURE FOR ENSURING SAFETY WHEN PERFORMING WORK WITH INCREASED HAZARD

High-risk work includes work that requires high consistency in the actions of workers, the implementation of special technical and organizational safety measures, as well as constant monitoring of the progress of work to perform it safely.

The list of work with increased danger, the performance of which requires preliminary training and testing of workers’ knowledge on labor protection issues, is approved by the chief engineer of the enterprise.

The enterprise has established a system of work permits for performing work with increased danger.

A work permit is a written permit to carry out work, which indicates the nature of the work, the place, time of start and end of the work, the conditions for its safe performance, the composition of workers and persons personally responsible for the safety of the work specified in the work permit.

A permit is issued before the start of high-risk work.

The right to issue work permits is granted to persons who have passed labor protection certification.

The list of such persons is approved by the head of the enterprise or the chief engineer (technical director).

Supervisors, mechanics and power engineers of the workshops where these works will be carried out have the right to issue work permits in accordance with the specified list.

For work with increased danger for general plant purposes, permits are issued by the chief engineer (technical director) of the enterprise or his deputy, chief mechanic or chief power engineer.

The person who issued the permit is the responsible manager of high-risk work.

The work permit provides for safety measures during work. If necessary, sketches of protective devices and devices, diagrams of plugs, placement of cordon posts or warning signs, etc. are attached to the permit orders.

The work permit is issued to the responsible manufacturer (performer) of the work.

In exceptional cases (the need to carry out work in order to prevent an obvious accident, eliminate a threat to the lives of workers or eliminate an accident in the initial stage), a work permit may not be issued, but the work must be carried out under the mandatory supervision of the responsible engineering and technical manager.

The responsible manager of high-risk work is obliged to personally instruct the responsible producer (performer) of the work on the contents of the work permit, check the implementation of all safety measures specified in the work permit, and subsequently monitor compliance with safety measures during the work and after its completion.

The work permit is issued again if, before the completion of work under this work permit, the nature and scope of the work has changed, which has caused a change in the conditions for their safe execution, the composition of the team has changed, and also if the provided safety measures have turned out to be insufficient.

During breaks in work during the working day (lunch break, break due to technological conditions of work), the work permit remains in the hands of the responsible producer (performer) of the work.

After completion of work under the permit, the workplace is put in order, and the permit is handed over to the person who issued the permit (responsible work manager).

Interrupted work can only be started after a new permit has been issued.

Closing of the work permit after completion of the work is formalized with the signatures of the responsible manager and the performer (manufacturer) of the work.

The closed permit is returned to the person who issued it and is stored for a year.

The person who issued the permit is responsible for the instructions and safety of the work, the correctness and completeness of the safety measures specified in the permit, disconnecting the repaired area from energy sources and pipelines with hazardous materials; human health with substances, compliance with the qualifications of performers; assigned work.

The responsible executor (manufacturer) of the work organizes the technical supervision of the work, compliance with the safety measures specified in the work permit, safety rules, special instructions, and instructs workers.

Admission rules

This document does not have a strictly established, mandatory template, so it can be drawn up in free form or written out according to a template developed and approved by the organization. The permit is filled out either on a regular A4 sheet or on the company’s letterhead in two copies. One of them is transferred to the work manager, the second to the employee who is responsible for fire safety at the enterprise.

Work permit for hot work- a permit document that clearly states the facility where the work will be carried out, its name, information about the specialists receiving access to it, the persons in charge, as well as fire safety measures taken at the workplace.

According to the rules, a work permit must be issued for all types of work separately for one day shift.

POT RO 14000-005-98 "Regulations. Work with increased danger. Organization of implementation" was developed by the Engineering Center for Safety in Industry.

For work with increased danger, in which several workshops and services of the organization take part (for so-called combined work), permits must be issued by the chief engineer (technical director) of the organization or, by his order, his deputies or chief specialists of the organization.

For high-risk work performed by contractors, permits must be issued by authorized persons of the contractors. Such permit orders must be signed by the appropriate official of the organization or workshop where these works will be performed.

For work of a local nature with increased danger, the issuance of work permits must be carried out by the heads of departments and their deputies where these works are to be carried out.

Those responsible for safety when performing work according to work permits are:

1. The person issuing the permit;

2. Responsible work manager;

3. Responsible worker (Observer);

4. Permitted to work;

5. Team members performing work according to the work permit.

PROCEDURE FOR REGISTRATION OF PERMISSION ORDERS

A work permit to perform work with increased danger must be issued before the start of this work.

The work permit should be issued in the department where high-risk work will be performed. The issuance of a permit must be recorded in a special journal.

The work permit can be issued for one shift or for the entire period of work performed in a continuous manner with an extension for each shift and issued to one Responsible Work Performer (Observer) with one team. The extension of the work permit must be drawn up and carried out by the Person Permitting to Work before the start of each shift.

The permit must be written out in two copies and filled out with legible notes in ink. Text correction is not allowed.

When performing work with increased danger by two or more teams at one site, a permit must be issued to the Responsible Work Performer for each team, signed by one person. When issuing work permits, measures must be developed to ensure the safety of workers, taking into account the joint nature of work performed by teams.

Production areas, technological lines, etc., where the production process has completely ceased, as well as buildings and structures located outside the existing divisions of the organization, allocated for the performance of work with increased danger by a contractor or other department, must be for production works were transferred to them according to the act. In this case, issuing a permit and ensuring the safety of workers is entrusted to the administration organizing work with increased danger.

If existing current, steam and gas pipelines, etc. pass through the facility where work with increased danger is expected to be performed. or overhead cranes are operating, then such an object cannot be transferred to the manufacturer of the work under the act for carrying out work on it with increased danger.

Work carried out near existing power lines and hidden communications must be previously agreed upon with interested organizations, and the relevant documents (communication diagrams, etc.) must be attached to the permit.

PROCEDURE FOR PRODUCING WORK WITH JIMS SELF-PROPELLED CRANES NEAR AN OVERHEAD POWER LINE

Work with jib self-propelled cranes at a distance closer than 30 m from the lifting extension of the crane in any position, as well as from the load to the vertical plane formed by the projection onto the ground of the nearest wire of an overhead power line energized by 42 V or more, must be carried out according to - approval (Appendix 13), which determines safe working conditions.

The procedure for organizing work near power lines, issuing work permits and instructing workers must be established by order of the organization.

Safety conditions during work specified in the work permit must comply with the requirements of GOST 12.1.013.

The validity period of the permit is determined by the person who has the right to issue permits, within the limits determined by the order of the organization.

The permit must be issued to the crane operator before starting work.

The crane operator is prohibited from unauthorized installation of the crane to work near the power line, which must be recorded on the waybill.

The work of the crane near the power line must be carried out under the direct supervision of the person responsible for the safe performance of work by cranes, ensure the implementation of the measures provided for in the work permit for safe work conditions and make an entry in the crane operator’s logbook about the permission to work.

When carrying out work in the security zone of a power transmission line or within the boundaries of breaks established by the rules for the protection of high-voltage electrical networks, a work permit can be issued only with the permission of the organization operating the power line.

The operation of jib self-propelled cranes under undisconnected contact wires of urban transport can be carried out provided that the distance between the crane boom and the contact wires is maintained at least 1 m when installing a limiter (stop) that does not allow reducing the specified distance when lifting the boom.

The procedure for operating cranes near a power transmission line made with a flexible cable is determined by the owner of the line. In this case, issuing a permit is not required.

PROCEDURE FOR COMPLETING THE PERMISSION ORDER

Entries in the work permit for work in electrical installations (hereinafter referred to as the work permit) must be legible. Correction of the text is prohibited. The numbering system for permit orders is established by the person responsible for the electrical equipment of the organization.

In columns that do not need to be filled in, you should put a Z sign, and in the lines you should make a dash.

The "date" line contains the day, month and last two digits indicating the year. For example - 10/03/95.

The surnames of the persons indicated in the permit are accompanied by initials, and for the Permit, Responsible Manager, Work Foreman, Supervisor and team members, their electrical safety groups are also indicated.

The line “Division” indicates the structural unit of the organization (workshop, service, area) where work is carried out under the permit.

In the line “Responsible work manager” the Last name, I., O. is indicated; if the work is carried out without him, “not assigned” is indicated.

In the lines “with team members” the composition of the team is indicated by name (except for the Work Foreman and the Supervisor). Surnames are written in the nominative case.

If there are not enough lines, a list of team members signed by the person issuing the permit should be attached to the permit, which should be written down in the last line “See additional list.”

The “assigned” lines indicate the names of electrical installations and connections where work is to be done, and the content of the work. For an overhead power line, indicate the name of the line and the boundary of the area where work is to be done (number of supports, individual spans, etc.), and the content of the work. For a multi-circuit overhead line, the name of the circuit is also indicated, and in the case of phase-by-phase repairs, the location of the phase on the support.

The line “Complete the work” indicates the date and time of completion of the work for this work permit (regardless of the completion of the entire work as a whole).

In the lines “Separate instructions” the following is recorded:

Stages of work or individual operations that must be performed under the continuous supervision of the Responsible Work Manager;

Permission to temporarily remove groundings;

Appointment of persons responsible for the safe movement of goods by cranes;

In the lines “Issued work order” and “Extended work order”, the issuing work permit indicates the date and time of its signing.

After complete completion of the work, the Work Foreman signs on the line of the work order intended for this purpose, indicating the time and date of execution. The Responsible Work Manager also signs in the corresponding line after he has accepted the workplace. If the Responsible Work Manager has not been appointed, the Work Foreman signs on the line and for him.

The person who issued the permit monitors the correctness of the clearance order and signs for its closure.
